What to Expect
Before Your Visit
If you are a new patient, you can download our new patient forms from our website. Here, you’ll be able to fill out information about your insurance, dental history, and previous medical history. This information helps Dr. Foote choose the best treatment plans based on your particular needs. If you are unable to fill out the forms ahead of time, it is important to arrive a few minutes early to complete this important information.
If you have TMJ problems, you’ll need to fill out our Temporomandibular Disorder questionnaire.
The information shared on these forms is kept confidential. If you are a returning patient, you will want to inform Dr. Foote and his staff of any recent changes regarding your healthcare information or medical history.

During Your Visit
You can expect the following procedures during a typical dentist appointment:
Dental examination – Whether you are coming in for a regular checkup or because of tooth pain or discomfort, we will start with an examination of your teeth and gums. Dr. Foote will thoroughly look over your teeth and identify any cavities, periodontal disease, or any other problems. X-rays will be necessary for a comprehensive evaluation of your condition.
Teeth cleaning – During your visit, we may provide you with professional teeth cleaning to remove any tartar or plaque that has developed on your teeth. This is a safe procedure that prevents tooth loss and periodontal (gum) disease. It is recommended that you get your teeth cleaned at least twice every year. After Your Visit
After your treatment, Dr. Foote will provide you with instructions for caring for your teeth post-appointment. It is important to follow these instructions closely to prevent complications or damage to your teeth or gums. If you happen to experience any abnormal pain or discomfort after your visit, please contact our office right away.
