You might notice some new forms which are present on this site. Specifically, the dental history is designed to more thoroughly address patient concerns and assess risk factors to one’s oral health. By doing so, we can treat people in a more thorough manner and help them maintain health throughout life. The “TMD” questionnaire is designed to evaluate a person’s jaw joints if there is some concern or problem. All of these forms can be filled out online and emailed back to us or printed at home and brought in to your appointment.

These forms can be found on our website’s Forms page. Our hope is that filling them out will not take up more than 15-20 minutes of your time.
